Roof and 4-Point Inspection Form Updates

Citizens has updated the following forms to better evaluate risks and determine whether a property meets Citizens’ eligibility guidelines:

Agents and Florida-licensed inspectors should begin using the updated forms immediately. If an agent submits another version of the inspection form(s), it will not be rejected. However, Underwriting may reach out to the agent regarding information missing from the form.

The previous versions have been deleted from our website, and the updated forms are available at:

  • The Agents website: Select Personal → PR-M → General
  • The Public website: Select Insurance → Inspections


4-Point Inspection Form

The changes on the 4-Point Inspection Form are outlined below.

Section of 4-Point Inspection Form Description of Changes
Minimum Photo Requirements Photos are now required to show the temperature pressure relief (TPR) valve on the water heater.
Electrical System
  • Clarified that separate documentation of any single-strand aluminum wiring remediation must be provided and certified by a licensed electrician.
  • Additional types of wiring have been added to the Supplemental Information section.
HVAC The Hazards Present section separates a single question into a two-part question:
  • Is a wood-burning stove or central gas fireplace present?
  • Was it professionally installed?
Plumbing The Supplemental Information section has been updated to include the following:
  • Age of piping supply system
  • Age of water heater
  • Age of piping drain system:
  • Original to home
  • Completely re-piped
  • Partially re-piped
  • Additional pipe types have been added
  • Year installed of PEX
Roof Any visible signs of leaks must be expanded if Yes is checked.


Roof Inspection Form

The Roof Inspection Form (RCF-1 03 25) has been updated to mirror the roof condition requirements listed on the updated 4-Point Inspection Form.
